Thesis Completion Form (PDF file)

The Thesis Completion Form should be given to your thesis advisor. This form serves as a record of completion of the defense. It must be signed by the chair of your doctoral committee.

It is the thesis advisor’s responsibility to turn this form in to the Graduate Student Coordinator.

All the committee members must sign the front page of the thesis (printed on the appropriate paper, see Doctoral Candidate Thesis Checklist Word file) and the original and copy must be given to the Graduate School.
